About this client/child:
Jack aged 19 enjoys music, films and socialising. He especially enjoys attending Nottingham Forest Football with his father. Jack has cerebral palsy which effects all four of his limbs, a learning disability and epilepsy.  He is non-verbal (he uses eye-gaze and a talker at college) but can communicate his basic needs to those that know him. Jack requires full support with all his care needs.

Overview of role:
As Jack’s Support Worker your role includes delivering personal care, administering medication, moving and handling, feeding, accessing social and leisure activities and carrying out therapy programmes. Training in specific duties will be provided. The role will involve some domestic work. The family home has been fully adapted with ceiling hoists and swimming pool. A car can be provided for you to use when on duty. The family can offer at least 28 hours a week. Some flexibility will be required depending on Jack’s College schedule which could lead to a longer day on the Thursday and Friday. *The specific shifts that need covering are:

• Monday to Wednesday: 7.00am to 9.00am, then 5.00pm to 9.00pm
• Thursday and Friday: 10.00am to 3.00pm/8.00am to 6.00pm/10.00am to 5.00pm

Who this job would suit:
The family’s location is very rural-poor and there is poor public transport so someone who lives locally and has their own transport would be preferrable. As Jack’s Support Worker you should have a high level of initiative and be able to work as part of a team. It would be great if you had an interest in sports and are confident to accompany Jack in the swimming pool.

This is a great opportunity to work closely with Jack and continue assisting with his on-going development. You will receive bespoke training and will receive 1:1 support from the Case Manager working with Jack.
