Overview of role:
The role of Associate Case Manager requires you to establish and complete, under the supervision and in collaboration with professionals, a detailed initial assessment of the care, rehabilitation, and case management needs of current, new and potential clients including risk assessments, outlining a clear case management plan and objectives for that client.
