About this Position:
An established and expanding Case Management Company working in the Medico-legal sector is currently recruiting an Associate Case Manager to join their dynamic team. Our client works with children and adults with acquired / traumatic brain injury, spinal injury, limb loss, dementia, and polytrauma across London and the South East Counties. Case Management is a challenging but extremely rewarding role and is both varied and collaborative. An Associate Case Manager is responsible for meeting each client’s individual needs; from assessing rehabilitation needs, recruitment, and managing care packages to supporting families through the litigation process.
Overview of role:
The role of Associate Case Manager requires you to establish and complete, under the supervision and in collaboration with professionals, a detailed initial assessment of the care, rehabilitation, and case management needs of current, new and potential clients including risk assessments, outlining a clear case management plan and objectives for that client. The Associate Case Manager will provide ongoing case management support to the client, their family, and care team, liaise with therapists, medical practitioners, solicitors, social services etc. Most importantly you will act as an advocate for your client and ensure they are receiving the right support in all aspects of their care plan. This is a self-employed role offering £25.00 - £30.00 gross per hour, depending on experience. Your caseload will build as your knowledge of the role increases.

Our client is looking for a candidate with experience within the disability/care sector, in a senior role. This is a client-facing role so you must be a people-person, have an engaging personality, and be extremely resilient, and tenacious. As you will be managing your own caseload, you must be organised and confident in working independently. Excellent communication, IT, and written skills are essential. The position requires you to travel to clients’ homes and to various appointments, a driver with their own car is essential.
